About the Cook Children’s Community Health Needs Assessment
Leader Survey Purpose ETC Institute administered a Community Health Needs Assessment Leader Survey for Cook Children’s and the Center for Children’s Health. The purpose of the survey was to gather information from leaders about children’s health issues in Cook Children’s eight county service region. The data collected from the survey will help the CHNA research team better understand the concerns and issues community leaders have regarding children’s health.
Parent Survey Purpose ETC Institute administers a Community Health Needs Assessment Survey for Cook Children’s Health Care System. The purpose of the survey is to gather information from parents of children ages 0-17 years living in Cook Children’s eight-county service region. The data collected from the survey will help Cook Children’s and the community better understand the current state of children’s health and better focus on improving the health and wellbeing of children in the region
